Dr. Lavanya Krishnan MD, FAAD

Lavanya Krishnan, MD founded Arya Derm in 2017 after three years of practicing dermatology in San Francisco. A board certified dermatologist specializing in medical and cosmetic dermatology, Dr. Krishnan strives to provide patients with high quality medical care. Her approach combines cutting edge treatments backed by science with a detailed understanding of patient needs.

Originally from Atlanta, Dr. Krishnan completed her undergraduate studies from MIT and attended medical school at Medical College of Georgia and completed her dermatology residency at New York Medical College. When not in the office, Dr. Krishnan enjoys traveling and exploring the world with her husband and three children.

Dr. Forum Patel MD, FAAD

Forum Patel, MD joined Arya Derm after completing a prestigious fellowship in Cosmetics & Laser Surgery accredited by the American Society of Dermatologic Surgery at Union Square Laser Dermatology in New York City. Formally trained in medical and cosmetic dermatology, Dr. Patel is passionate about helping her patients improve on their already existing foundation to achieve the best version of themselves. She also believes that preventive care is fundamental to healthy aging when it comes to skin care and cancer screening.

Originally from Maryland, Dr. Patel completed her undergraduate studies at the University of Maryland, College Park and she attended medical school at George Washington University. Following medical school, she spent a year in Boston completing an internship in Internal Medicine at Tufts Medical Center and then moved to the west coast to complete a two-year research fellowship at the University of California, Davis studying autoimmunity and leading industry-sponsored clinical trials in psoriasis and pemphigus vulgaris; a rare but devastating disease with high mortality rates. She remained at UC, Davis for her residency and became Board Certified in Dermatology in 2018. She then went on to complete her Cosmetic & Laser Surgery Fellowship in Manhattan before moving back to California.

An avid runner and travel enthusiast, Dr. Patel firmly believes that the key to healthy skin and graceful aging starts with staying active regularly, whether it be working out in a gym, cycling around Golden Gate Park, or hiking the trails of Patagonia.

Insurance

Does your office accept insurance?

Yes. We strive to be as accessible as possible and are in-network with select insurance plans. It’s important to note that insurance only covers services that are “medical necessary.” Thus, cosmetic services are not covered by insurance

Which insurance plans do you accept?

We’re proud to work with Medicare and the following PPO plans:

What if I have an insurance plan not mentioned above?

If you have an insurance that’s not included in the above list, then we’re most likely an out of network provider. You’ll be responsible for the full cost of your visit. We’re more than happy to submit claims to your insurance carrier on your behalf for potential reimbursement. If your plan has out of network benefits, we’ll refund you for any reimbursement we receive. If you have  Health Savings Account (HSA), medical services are HSA-eligible expenses for reimbursement.

I’m plan to use insurance. What is my out of pocket cost?

Although we do our best to check your insurance eligibility and verify your benefits prior to your appointment, we can’t guarantee that the information you receive will be accurate. Ultimately, it’s your responsibility to understand your insurance benefits and your insurance company will determine your patient responsibility after processing your claim. We’re always here to help, but if you need specific information about your plan and benefits, we recommend that you contact the phone number on the back of your insurance card. If you have any questions about this, feel free to email us!

I don’t have insurance. What is my out of pocket cost?

Fees depend on the nature of your visit and whether any procedures are performed. For new patients visiting us for the first time, the consultation fee is $250. For existing patients, the consultation fee is $150. Procedures or biopsies are performed during your visit will incur an additional cost and we will discuss them with you prior to performing them.

Appointments

How do I book an appointment?

It’s easy – request an appointment online or give us a call at 415-329-5100. We’re happy to help!

How long should I expect my appointment to be?

The length of your appointment depends on the nature of your visit. Most standard consultations are 15 minutes in length. Quick visits, like prescription, follow-ups and suture removals may be a bit shorter. Procedures that are more involved – like surgeries, cosmetic treatments or excisions – can range anywhere from 30 to 90 minutes.

Do you offer weekend appointments?

At this time, we’re only open on weekdays.

What if I’m late?

If you are more than 10 minutes late to your visit, we’ll do our very best to accommodate you. We hope you understand that you may have an extended wait time. In the event that we can’t accommodate you, we’ll help you reschedule to the soonest available appointment time.

Policies

Why do you need my credit card on file?

We keep a credit card on file to make your experience seamless and to save trees. Oftentimes, there is a small balance remaining after a visit which is not completely covered by your insurance provider.

It’s no different from keeping your credit card on file with Uber/Lyft or providing your credit card when you make a hotel reservation. We will always email you an invoice explaining these charges so that if you prefer to pay with a different card, you can do so online. Otherwise, we’ll charge your credit card on file after 5 days.

For cosmetic services, payment is due in full at the time of service.

Do you store credit cards securely?

Absolutely. Credit card information is stored in our PCI-compliant payment gateway, which employs the most stringent level of certification available in the payments industry. Once entered, our staff cannot access your credit card number – only the last 4 digits are visible. You’re of course welcome to change the card on file at any time, just let us know.

What is your cancellation policy?

In order to preserve the integrity of our schedule and to reserve each individual patient’s full appointment time with our doctors, we require that you notify us at least one full business day prior to your scheduled appointment if you need to cancel or reschedule. We charge a $100 dollar no-show fee if you miss your scheduled visit and fail to notify us one full business day prior to your scheduled appointment. For example, if your appointment is on Monday at 10AM, we require that you notify us by Friday at 10AM in order to avoid the cancellation fee.

Why do you charge a $100 no-show fee?

Trust us, the last thing we want to do is collect a no-show fee. We’d much rather you show up for your scheduled appointment or, if something comes up, notify us at least 1 business day in advance. As a small, startup practice operating in an expensive market, it’s critical for us to maintain the integrity of our schedule. We never double-book appointment slots. When we receive advanced notice from you that you can no longer make your appointment, it allows us to accommodate patients with urgent medical needs. No-shows prevent those patients from being seen. We thank you in advance for your understanding and cooperation.

How long we retain your data
